Original Medicare covers 80 percent of your approved medical costs. You are responsible for the other 20 percent with no out-of-pocket maximum. For a major surgery or a long hospitalization, that 20 percent can be a significant number.
A Medicare supplement plan, also called Medigap, fills in that gap. Depending on the plan you choose, it can cover the 20 percent coinsurance, hospital deductibles, skilled nursing facility coinsurance, and in some cases foreign travel emergencies.
The most comprehensive plan, Plan G, covers almost everything except the Part B deductible, which is $257 in 2025. Most people who choose Plan G pay a monthly premium and then have essentially zero out-of-pocket costs for covered services for the rest of the year.
The premium for Plan G varies significantly by age, gender, location, and the insurance company offering it. A 65-year-old woman in most states might pay $120 to $180 per month. A 75-year-old man might pay $200 or more.
The trade-off versus Medicare Advantage is that Medigap works with any doctor who accepts Medicare, anywhere in the country, with no network restrictions. For people with complex health needs or those who travel frequently, that flexibility has real value.
